The slide label provides nine lines of information as follows:

OPD name and accession number
Patient's name
Patient's ID number at the doctor's office
Specimen site
Date biopsy was obtained
Date sample was received at the laboratory
Medical clinic's name
Client's slide accession number
Physician's name
